You’re asking about Jamestown Scuba owned by Bob Clark. Ocean State Scuba was opened during the 1990’s. Both are closed. Bob Clark’s whereabouts is unknown to me.
You’ll probably need to go to whatever agency your cert is from.

Bob is still around. I see him in town from time to time. He is mostly into golf now.
 
Were you successful in replacing your card? If not, please feel free to contact me and maybe I can help. I don't have any records anymore, but may be able to help you recall instructor names , dates, etc...
