I had an OOA incident a couple of days after I got my OW certification.

My primary reg loosened on the hose, and started to free flow. Hand-tightening the reg did not help stem the free flow. Nothing helped. I switched to my octo. My wife and I started an ascent from about 50 feet.

That continuous free flow was a HUGE distraction. And it made me really nervous.

During the safety stop I was nearly OOA, so I switched to my wife's octo. We finished the safety stop and climbed aboard the boat.

In retrospect, I should have reached back and turned off the tank while there was still a couple hundred p.s.i. in the tank.

Well safety stop is not a requirement if you have not gone into deco. So next time skip the safety stop.( assuming you have not gone into deco :) Using your buddies octo also fine.
