I just don't log dives
I thought this was an amazing inquest: Service log for scuba gear used by British music producer who died off Byron Bay source of confusion at inquest

I can't remember the last time I logged when I serviced individual regs, bcs, etc.

I log in a spreadsheet service to all of my regs, tanks, and computers. I have records all the way back until I started diving. I capture date, who did the service, and cost.

I do not log service to wings because I don't do anything beyond regular inspection and replacing the inflator when they become gummy and the exhaust valves when they develop issues or when the strings break.
 
1) it is Oz and they seem to be safety (over-)regulation happy over there now. (ex. yearly hydros)

2) service records for "life-safety" equipment used for commercial rentals does not strike me as unexpected. If I were an insurer, I would demand it as a condition (service according to manufacturer's spec and records).
 
Was there an equipment failure in this case?
I couldn't suss that out. The last line was "A finding was expected to be handed down by the end of May."

I've never been, I wouldn't know.
