I just don't log dives
I completed PADI DM with a shop in Greece in July and have since moved back to the States. I have been inquiring as to the status of my application and have found that it is on hold due to the hologram being missing from my application the shop sent it. I don't have the hologram or a receipt to prove I bought the materials. The shop is not responding to my attempts to contact them in order to fix the problem. The regional office says to mail in the hologram or receipts which I do not have. I contacted Quality Management and was told to contact the regional office. Any advice?
 
You do not mention if you actually DID purchase the materials.

If you did so, by credit card or check, perhaps you can get a credit card statement or cancelled check?

Is the shop still in business? Perhaps their regional office representative can contact them; they may better respond to PADI's call over yours.
 
Thats a tough situation.....without the hologram, PADI will not issue a cert....when I did my DM, the first thing they did was give us the hologram and let us know how important it was........our instructor told us outright that he did not want to be responsible for them......

we then gave it back at the end of the class so that out papers could be mailed in...

Funny, my NJ-based LDS never mentioned "hologram" importance. In fact I'd never actually heard of such a thing until this thread.
 
The hologram is needed for the "new course". I did the old course but finished well after the date PADI declared for needing the hologram if you didn't finish by then. Somehow I got the card without the hologram, probably due to my owner's inginuity.
 
Apparently the dive shop closed for the winter, weird as you can dive there year around. The QM realized it was not my fault will let me certify and will take issue with the shop when they resume business next season. Future DM's make sure you keep at least a receipt to prove if the hologram goes missing. Greece is primary a cash economy so credit card records are usually no available.
