Good point. Did check hoses, valves and such, never occurred to me to check tie-ons, until I lost my lamp. Now, it is on my "always check" checklist
 
I use cave line, three loops. Never had any break but I do use new cave line a re-tie annually ditto bungee. Prevention and all that...I also add a drop of glue to the knot.
 
Here's a couple picture of how I secure gear. Everything gets a cave line tied to a bolt snap and smaller items also get these perfectly sized (and priced) coil lanyards from Piranha Dive Mfg. Lanyard-Black-Coiled-w-SS-split-rings (Light, Knife, EMT Shears, Camera Rig). My diving tends to require quick transitions from tasks so, for example, I may need to drop my light to secure something else so the lanyard works great, for others it may not be necessary.

Here's my shoulder D ring mounted light. Cave line tied bolt snap (second pic shows how I tie cave line) to butt of light, coil lanyard secured to bolt snap underneath light head back to D ring with a plastic clip. Using plastic clip avoids mistakenly releasing the wrong bolt snap. Finally the light head is secured with a length of shock cord that goes through the ring where my chest strap and waist webbing attaches.

Here's how I tie cave line to every item. Three loops secure the item and three loops secure the snap. This technique allows for a very secure rigid line, yet still can be cut by shears or a line cutter. It also allows custom lengths depending on what ever it is I want to secure. This picture is of the grip of my speargun where I needed a longer length.

The light I lost was secured with 2 loops of brand new cave line. The light was less than 2 months old, so I had just tied the bolt snap on within 2 months. The knot was partially melted, rather than glued, and it was still tied to the bolt snap, which was still haning from my chest D-ring. The new cave line simply got sawn through by the inner edge of the hole on the butt of the light.
 
Doesn't seem like such a great deal now.... :-\
I dunno.... It'll be a great deal for the guy who finds it... :rofl3:

Didn't read the whole thread properly, sorry. I understand. I had a couple of older OMS lights and Intovas that had very sharp edges. I ended up filing them down.
