No need for an adapter. I don't make a lot of calls when I am there, but I have always just used the hondutel office. I don't recall the price, but I seem to recall its pretty cheap. UTLgirl can probably tell you the best solution if she checks in.
 
For calling home there are a few places with VOIP, but they were not all that cheap. I did have to use one. It's OK for a single quick call per week.

In at least one of the internet cafe's Skype was on at lest one of the machines. You would need to set up an account before you leave home and be careful to log out when using Skype. You might need your own headset. I have never tried it that way, but it should work.
 
Looking at Treetanic website it seems they have WI-FI now.
