I once booked a trip for someone who proposed to their girlfriend underwater. They took a picture and sent it to me. It was really great. The guy arranged everything before hand so everyone knew what was going to happen except her. The biggest thing was getting the signals down so the camera guy knew when to get ready to take the picture. So there's my advice. Figure otu the hand signals for "go" time.
