I have decided to get my cardiologist to just write a short letter stating that he is my doctor and he has cleared me for diving.

I much suspect that a short letter from your cardiologist stating that he is your doctor & has cleared you for diving generaly will serve well, although not in Australia, Malta & couple of other venues.
