Not that you shouldn't try DAN doc, by all means try all the avenues open to you....but sometimes a trick of the trade can also be helpful....dependent on your circumstance of course......
I also had tremendous trouble equalizing, including one side taking way longer than other side. I couldn't go down without tremendous pain. I remembered reading somewhere on SB that some people would equalize on the surface. I tried that, and much to my surprise, I literally stopped having pain. Just swallow and blow your nose repeatedly 5 minutes before you jump into the water, and usually that'll solve your problems of equalizing.
A couple of time that I would forget to equalize on the surface, I had trouble equalizing underneath, so this really drove the point home how helpful it was to do it on the surface before descending.
Believe me, I was so glad to finally solve the problem since I was really worried about how much pain I would have, and a couple of times my hearing was muffled after diving, and this really scared me.
You don't state anywhere in your posting of equalizing on the surface...try it. It'll hopefully help your equalization problems or not, but it can't hurt.
