When it comes to accountability of heads on a boat,
"K I S S"

Everyone signes in before boat leaves the dock.
After the dive, DM or whom ever, does roll call. No one answeres for anyone else. If so and so is down taking a nap below, the roll call stops until so and so is present and accounted for in person.
 
The tags can't fail, assuming the boat captain follows the rules of

"Nobody goes in the water without a tag"
and
"The boat doesn't leave until all the tags are back"

Everything else is easy to fake out (even unintentionally).

A buddy can write down his friends tank pressure and even sign the sheet.

The boat captain typically wouldn't be examining signatures for a forgery, however tags are pretty cut-and-dried. Either you have a whole board full of tags, or you don't.
