Thats fine, I made a mistake myself when posting; should have been "but, ^no^ sure ideas".
I would definitely agree with you that any pressure in the tank that is even close to 28 psi wouldn't be enough to move the diaphragm in the first stage, but according to my instructor, the diver had something like 1000 psi remaining, he was Solo and he was nearing old age, especially for a diver. Sounds like there were more than one contributors to this tragedy. Live and learn, hopefully!!!
