Stuck on the way out is my guess.
If he got stuck in the revelation space restriction and took it off to pass that restriction of course it was found there. My hunch is that he pushed it in front of him to get out, then lost contact with it.His CCR was in the Room of Dreams, far from the next restriction on the way out, which would be near the Pit, if I would venture to guess.

So, why would he remove the good & functioning CCR then?
Either stuck on the way out, or on the way in with an abort & turn-the-dive call.Stuck on the way out is my guess.
Maybe he was pushing the 95 and pulling the rebreather et al?
Fwiw you don't really have to be in constant contact with gear when negotiating a restriction. It's not going to go anywhere since the space is so small.That's a good possibility. Just thinking of how he managed to hold the 95 bailout bottle on one hand & push the CCR with the other hand thru the restriction.
