Does anyone have a pre-done fillable tech dive plan format to leave a dive plan with a boat crew/captain. I could jin something up but would rather plagerize if someone has already done the work.

Most dive planners have an output option. Why not use that?

As a long time tech diving captain, I approve this plan. Any other information is either redundant or extraneous.
Additionally I’d like you to shoot your DSMB upon leaving the bottom or arrival at the first stop. But we can negotiate that if you have reason for another plan.
Looks like Capt. Jim Wyatt agrees as well.
