I just don't log dives
If you have DAN dive insurance on a live aboard and incapacitated during a dive accident, who is calling Travel Assist to arrange and pay for your evacuation as required by the policy?

Isn't DAN insurance policy # one of the blocks you're supposed to fill in upon arrival or during booking? If that is the case then the operator should be calling.
 
On a LOB the Captain will most likely be involved.

Standard practice on the LOBs we have been on is to provide your insurance information (DAN or otherwise) as part of the onboard registration process.
