Custom Dive Log Binder for a gift

Hi,

I am wanting to get a present for my girlfriend, and she really needs a book to keep her dive logs in. I was hoping to have a custom leather binder made for her, and make my own dive log pages to go inside, and the first page inside the book is going to be a charicature of her scuba diving.

Something like one of these:

And I was hoping to have something stitched on the cover to personalize it. Something simple like "Jane Doe's Dive Log" but in big, fancy font.

Has anybody done something like this, and if so, do you have any recommendations about how I should proceed?
