Chart Wreck Identification?

Nautical charts show wrecks as a rib cage type icon. What is the method for finding specific information on the located wreck. Is there information somewhere for every wreck that has an icon? Also will an icon be removed from a chart if the wreck has been proven to have deteriorated to nothing?
