If it's predictable, then it shouldn't be a surprise. People who routinely live in denial will be hurt by their delusions one way or the other.

Actually Pete, the term Predictable Surprise was coined by Diane Vaughan who also gave us the Normalization of Deviance in her book about the Challenger and circumstances surrounding its ill-fated launch.
I like it because it makes us think... and when we think we might admit that something we initially viewed as unexpected is in fact a normal consequence of our action... or inaction.
