I own an Alkin W31 and the fill whip has this lever and valve that lets you bleed the air past the quick connect fitting while keeping the line pressurized so you can keep filling cylinders.
When i switched cylinders and flipped the lever to pressurize the tank the qd let go under pressure and slammed agaisnt the floor which broke the gauge.
I checked the ball bearings inside the qd fitting and they look fine so idk why this happened but it scared the crap out of me. I made sure it was secure by tugging on it before i pressurized it and theres no damage on the fill nipple either. Any clue how this could have happened?

The collar wasn't slide all the way towards the male part. The collar is what holds the bearings in place and prevents them from expanding outwards and releasing

I made sure it was.
