So if you decline coverage and get lets say a flat tire and the tire is ruined, whats the process? I have personally seen folks told to go find a garage and repair or replace the tire. I have been paying the full insurance at AB just to not have to deal with it. Just thought I'd mention it...

I had this happen to me last year. I think it was Total Car Rental. We didn't have the part of the jack that you put through the bumper to lower the spare tire (we had it jacked up, and the lug nuts loose, but couldn't access the spare). We called Total, and they quoted us an unrealistic amount (like over $500) for a new tire....and they would not change it for us. They did bring out an assortment of the tool we needed, since they weren't sure which fit which truck, so we could get the spare. Even though the group rents around 50 trucks a year from them, they wouldn't cut us any slack. They suggested we take the tire to a tire garage. We took it to one by the "other" gas station, not the old Lisa's and they dismounted the tire, patched it, remounted it (all by hand) and put it back on the truck and put the spare back in its storage, all for about $7.50. I think it was $10NAF, but I gave them $20USD, including tip, since I was happy not to have to pay $500 to Total. So, I'm not sure (it doesn't specifically state so) that tires are covered with the Standard CDW. They aren't in the USA either. Nor are they covered by my personal Auto/Truck policy. I do have road hazard insurance, but that is from the tire company, not the insurance company.
 
The insurance I speak of is the american express premium car rental insurance https://www295.americanexpress.com/premium/car-rental-insurance-coverage/home.do - it is frequently mentioned on bonaire talk, and does work with pickup trucks. Sounds great, doesn't it?

I completely understand that the built on to the card insurance doesn't cover everything, but, this seems to cover everything needed, as primary insurance, for far less than the car rental shops

I just rented a truck on Feb 19th on Bonaire using my Amex card with premium ins. so AMEX is still accepted by some rental agencies.

It is my "personal opinion" that several of the rental firms on Bonaire do not want to lose the lucrative ins income so have decided to not accept AMEX, which is one of the few or maybe the only credit card that covers pickup trucks. There have been numerous reports over the years about gouging customers for damages or tire repairs on Bonaire.

That said, about 10 years ago, I was the victim of a hit and run on my parked van in front of Kaarls beach bar and my Master Card came thru and paid for all the damages. I now use Amex because they cover Pickups and Master Card does not.

My advice is check around and find someone who will accept Amex. Their premium insurance cost about $25 for a forty day period versus about $19+ per day for the rental company ins. Do the math. Even if you have to purchase a used tire on your own dime, it is a great savings.
