Thats what happened- he had a runaway ascent. The cause? we'll never know. It looks as though he took an O2 hit on his ascent, as he was switching gases he may have taken the wrong reg. Such an easy mistake, it could happen to anyone.

Diver dies in Orkney
39-year-old Hillary Lyon from the Bolton area was found unconscious at the surface after a dive at Mull Head, Deerness on Thursday 15 July; she was later pronounced dead.
Dive charter boat Karin contacted the emergency services after the diver was spotted floating face down on the surface and was recovered into the boat. The woman was one of a party of divers visiting sites around the Orkneys, north of Scapa Flow.
Attempts were made to resuscitate Lyon, who was unconscious and had stopped breathing. She was pronounced dead by a local doctor who was rushed to the scene by the Kirkwall lifeboat
String:Solo diving isnt neccesarily a bad thing, a lot of the very deep stuff people dive in a very loose pair and are totally self sufficient in terms of gas and equipment. Solo diving doesnt *cause* an incident.
