Anybody, with the exception of U.S. citizens, legal permanent residents, and immediate family members, who has been in the following countries during the past 14 days may not enter the United States at this time:
China
Iran
United Kingdom (England, Scotland, Wales, Northern Ireland)
Republic of Ireland
Brazil
South Africa
European Schengen Region (Austria, Belgium, Czech Republic, Denmark, Estonia, Finland, France, Germany, Greece, Hungary, Iceland, Italy, Latvia, Liechtenstein, Lithuania, Luxembourg, Malta, Monaco, Netherlands, Norway, Poland, Portugal, San Marino, Slovakia, Slovenia, Spain, Sweden, Switzerland, Vatican City)

I do not understand this list. For example Malaysia and Indonesia are not mentioned?

Neither do I. From Asia, only India & China are under the US temporary travel restriction.
 
The problem with CoVid testing, a requirement prior to entering/returning to the US, is that, it does not catch 100% of the cases. There are quite a few variables- incubation period or timing of the sample, adequacy of sampling, number of virus (if present), etc. Best to just continue to practice social distancing, wear mask and practice good hygiene, irregardless of vaccination status.
I knew about that long time ago.
Many arrival from Indonesia and Philippines with -ve test result have been found to be infected.
The accuracy of the test is also questionable. And I would not be at all surprised that some of the "results" are faked.

Malaysia is irrelevant either way. The government no longer calls it Movement Control Order (MCO)/lockdown but Phase 1 which was supposed to end on June 28. However, since "we" failed to meet three target numbers the start of Phase 2 is delayed indefinitely until we magically reach those target numbers.

There are only 26 countries in Schengen Region. Monaco, San Marino and Vatican City are not included.
