My simple and very effective means of dealing with a primary wing failure diving wet is the addition of a redundant stacked wing,,,,dry diving the dry suit is my redundant....and in both gear configurations my lift bag which is always on board for the ride is a fall back.
 
Even with a bladder failure in most cases its possible to orientate your body so it keeps air. Absolutely catastrophic failures that leave the wing totally empty are exceedingly rare. I've never heard of it.

The most common (I've seen it a couple times) is for the corrugated hose to pop off the elbow. Generally seems to happen on the surface not UW
