I had three of them go two weeks ago when diving in Seattle. The first one happened when the tank was being filled, and the last one happened when my wife took off her valve. I consider those 'operator error'. However, the second one blew at the end of the dive when I was taking my BP/wings off on the surface. I don't know *what* happened for sure, but I suspect I bumped the valve with my head (not very hard mind you, since I didn't feel anything), and my wife noticed the unit bubbling and making noise.

I have to admit that having the O-ring blow under pressure (~800 psi) and connected was a bit disconcerting, and made me more motivated to get some DIN valves. (However, at this point I think the 232 Bar valves would work best for my situation, because of the unavailability of DIN fills locally.)

It's not like O-rings are expensive, but having three go out during the 2 days was a bit excessive. I suspect the rental tanks we were using had crappy/old O-rings in them, which is yet another good reason to have your own equipment. (I had two of my own tanks, and 4 rental tanks for the weekend, and my tanks had no problems, while 3/4 of the rental tanks were the ones who blew the O-rings).
