I finally lined up a instructor to get my open water cert. He as not taught a class using the self paced go dive crew pack course!!! What does he need to sign off to send to PADI?

If this instructor is the one who is going to certify you, the only thing he has to send to PADI is the PIC (student registration) envelope.

When was the last time he taught a class? If he's not sure what to send to PADI and hasn't used the crew pak, you may want to make sure he's a current, teaching status instructor before paying him.
 
He is my friends dad and has assisted in many classes. He is doing this as a favor and has not went through the reg process. Where can he get the PIC? I guess I am a test subject for the process.
 
if he is not in teaching status with current insurance you are wasting your time...if he is and he is signing the pic without documenting that you passed the academics and the open water portion of your training he is violating standards and can be suspended and or terminated from ever teaching again.. if he does not know what to send to PADI I very highly doubt he is or ever was a PADI instructor..even my divemasters know the process.
