what direction to turn adjustment knob

The orifice on a G250 (not the adjustment knob) should be adjusted so that you get minimum breathing resistance - just on the edge of a slight freeflow or slightly farther to where you have very slight freeflow with the adjustment knob screwed as far out as it will go. Either method will leave you a lot of room to crank it down in surge, current etc, and will ensure max performance.
 
Careful with that adjustment protocol - it will leave the lever "preloaded" in some cases, which will make the reg hypersensitive to surge or current, and cranking in the balance adjustment will only partially compensate for it.

If all the parts are in good shape setting the lever to JUST touch the diaphram should result in about 1/4 - 1/2" of H2O cracking pressure with the knob all the way out.
