Get the names of the people you deal with. Don't hand money to any immigration employee who "asks" for it. When you pay (cashier), wait for the receipt.

Amen to that. I got a receipt from both Bohol Travel and an Immigration Office itemized receipt.
 
Don't worry, my money went only to the cashier - who was very helpful actually, explaining all the different charges. I actually have a one year multi entry visa, but the pain is that you still need to renew it every two months. The only advantage of the one year length is that it renews if you leave the country and come back in. I should probably have done that, as it might have been cheaper than renewing at the immigration office.
