Easy, fill whips and valves that are not blown out before filling. Most often a whip is put directly on a valve and neither are blown out first. Never know what may be between the valve / whip face and the seat. While remote, it is possible.
 
I guess I'm spoiled as I do my own fills on most of my tanks. I guess if a shop is careless, it could happen........but then maybe they shouldn't be filling then.
 
Again, what is the proof??? Agreed, IF there is a suspicion absolutely. Otherwise, if you are using O2 compat. air and not running tanks to zero how are contaminants going to enter?

Dragger tubes will quickly enlighten you. But hey, let me not wake you from your slumber.
