In that case a big chance that he may have been so preoccupied that he just ran out of air and not noticed till too late. Sad that there have been five scuba diving deaths now in the past few weeks in Australia, a huge number considering the diving population.
 
Not so sure about that Michael.
The TV coverage showed the hose real for a Hookah on the boat - though I couldn't tell for sure if it was the actual boat he was diving from.
If he was on surface supplied air then it's unlikely he ran out of air. Typically for the mass media, there was little useful information for accident analysis.
 
Without any hard data, anything about the incident is just speculation and wild guesses.

If hookah, then brings into the equation carbon monoxide.
 
Amazing, NSW does not do any reports at all unless there is a formal inquest. Even then, the reports have only been published on line in the past few years. Thanks for this link, some interesting reading.
