I have been debating this as well... My wife and I buddy up on most all dives. We have been discussing options for getting each others attention and I am considering an underwater laser. I figure that it is silent, effective by simply wiggling the dot on the reef or bottom within my buddies field of vision, and also makes pointing out that scorpion fish easier.

The problem I see with these is that you are rarely able to get someone's attention when they are looking away from you. You tend to focus out when you are diving and seeing a small red dot is sometimes pretty ineffective. Its not that easy to see.
 
Are there any surface signals that also work underwater?

The SubDuck says it "is not an effective surface signaling device."

What does a Dive Alert sound like underwater? How about a Hammerhead above water?
