Problem with revving the engine is that there is so much boat traffic down here, you'd never know if it was your boat, or just one going by. And, I don't think the ladder banging would travel far enough. Sometimes we anchor or tie up at a mooring ball on the reef, and then could swim hundreds of yards away from the boat u/w.

I was diving in the Keys when a storm was approaching the boat while we were diving. We ALL heard the banging on the ladder with a big wrench. It was LOUD.

As for a "regular" noise maker, I sometimes carry an abalone iron and use it as a tank banger. It's quite effective, too.
 
I just use a stainless clip banged on the tank. For a boat recall signal we bang on a tank held under water, but to distinguish from other sounds we use a distinctive rhythm.
