I use that form when traveling abroad. Just remember to have it signed BEFORE leaving. Every time I go to get it signed, the CBP officer looks at me like I’m out of my mind. However, when returning from Mexico, I was asked if I wanted to declare anything and I did not. They open all of the camera gear and told me I might be in trouble. I whipped out the form, they looked at each other kinda funny and let me pass.
 
I'm guessing that none of this is needed when traveling to another state. I might be going to Oahu for work next month and wonder if it would speed up going through security with this form.
I doubt it since Hawaii is part of the US. So there's no customs declaration.
Does everyone check their camera gear in or bring it on as an over sized carry on?
I carry it on. Over sized? What's that?

I have a backpack built specifically for my camera, maybe that would be the way to go and check in the housing, now where to put my laptop....

I'll be honest with ya Steve, I'll never sacrifice quality for size my friend. In our present time no single chip camera can come close to matching my 3ccd camera's performance, even if it is 4 years old :14:

You're allowed 2 personal items and I think it might be a pain to carry a backpack, laptop bag (also a backpack) and reg bag. I'm sure I'll figure something out.

When I was using a smaller video camera & housing I would put the camera in the housing and carry that on. It made for some funny looks at the airport, but I knew both were safe. Now with the Z1U the housing actually takes up two Pelican cases (housing & handles in one, port & lights & accessories in another). So I carry on the camera and check the cases. Be prepared to pay for the extra luggage, but I wouldn't think there would be a customs issue since you're travelling within the US.
 
I've never had a problem in the past, perhaps because both me and my equipment have that "mature" look (you know... well aged). If this is becoming a requirement, I guess I'll have to fill one out though.
