You were the one who posted that form, I just quoted part of it.

Right, I though you had quoted from the WRSTC form. The UKDMC is for members clubs where we have 3rd Party Liability Insurance. If you've made a false declaration on the medical form you can't claim against the club or fellow dive club members for loss of earnings etc.
