Adds a sense of urgency to getting the fracture treated (assuming it *can* be treated without knowing where it exists in the tooth).

It is definitely the tooth. I've been diving long enough to know the difference, the pain is acute (in that area). It would be easier to figure out what to do...if it was sinus pressure.
 
Wow, thanks fisherdvm. That was very helpful. I will get the tooth refilled, with porcelain and see if that takes care of it. The filling is old (I think), so it is definitely worth getting it refilled and see what happens. I know that I've had the filling for at least 10 years, and it hadn't bothered me until the past year.
