Depends on where in the world you are, too. In Australia tipping is not expected - but it is always welcomed (usually after some embarassed shuffling, though) as it truly expresses that you think the crew member did an outstanding job. I would NOT tip in Australia for "standard" service, only above and beyond.

In Cozumel, we tip each day directly to our crew.
 
I tip at the end of the trip 15% of what my diving costs. I let the dive shop divy the money up between DM, boat hand, and captain.
 
I usually tip 0-20% depending on the level of the service at the end of the dive. If we are on a group trip then I usually give the money to the person planning the trip and they give the tip to the crew.
 
I tip directly to the DM about 10 dollars a day (X2) when on vacation. Diving at home I don't tip anyone but I try and get some good pictures of the captains, the boats, etc and burn them a CD.

If it is a treacherous drift in bad conditions, I tip the captain before the dive and hint that there could be more.

I would just show him his tip and then carry in BC pocket till dive over :)

I tip every DM on every boat regardless of where in the world I am. Providing of course that a tip was deserved. Some places I've been to discouraged tipping, in cases like that I will not tip in cash but I'll buy the DM dinner somewhere.
 
I dont mind tipping when it is deserved. Unfortunately, in todays society a tip is expected for little or no service. Im sorry but in my world you have to earn your money, I do.
 
So do I give the tip to just the DM and assume he is going to divide the tip with the captain, or do I personally give the DM & Captain their tips?
