Interesting timing, Steve, as I just completed Gareth's survey about HFACS.

Started working on it at the end of September... triggered by the rash of incidents then. Shocking to me that during the four months it will take to get it from concept to published, there have been more than 10 deaths.

Perhaps you could change the title "In closing" to:

"In Closing - We can be heroes .. just for one day."

or something along those lines - that's how I feel sometimes when I hear about fatalities.
