As already mentioned, www.dive-logs.com do some nice logbook pages; I have the nature ones as I like recording which species I've seen during my dive. They have a great range of different ones, whether you're an instructor, photographer, interested in the species, etc, and whether you use a computer or not, and they come in both imperial and metric... You can also use their logbook designed to create your own pages. AND they come in standard size (such as for PADI binders) or smaller (they also sell binders, logbook stamps, ID cards, lanyards, etc...).

And of course, once you've found/desinged a lookbook page you like, you can scan it and print it... Much cheaper than buying refills! :)

Cave Diver:
I didnt like most of the logs I saw out there, so I made my own. Went to an office supply, and had the pages printed out and put a spiral bind on them. I took all the best information that I'd seen from other logs and combined it into what worked for me. Covers practically any diving I do, from rec to tech.

Now if only I'd use them more...

Ditto, I have found all "canned" logs lacking for my needs. With about 30 minutes on my word processor I created exactly what I wanted. My local print shop created 100 pages of double sided logs on a much thicker paper stock for $30.00. They also punched the holes to match my dive log book. It's handy for that annual trip to the live-aboard where they like to see your logs.

Cheers

I use Scubasehttp://www.scubase.net/ which lets me design my own pages, including info from my computer and/or Sensu Ultra and print them in the size of my choice. I use a 5x8 dayplanner cover since I already had a nice one and it allows me lots of options for storing additional items like c cards and business cards of folks that I meet on dive trips.

I scanned an SSI page and used it as a template in MS Publisher to create my own. I print them on glossy paper with a laser printer so the are somewhat water resistant.

I also found a pleather day organizer at Wal-Mart that uses the same size pages as SSI. I have it covered with dive stickers from places I've been. They stick really good to the pleather.
