after the first time they set up my gear I asked them not to. I am just super picky about where I want my BCD on the tank (nothing against them). I just tell them I will do it and they leave my stuff alone. Never had any really insistent/insistant that they do it.
 
I don't have an issue with the tank changes/set up part of it. As mentioned several times, it's YOUR responsibility to make sure your air is on, you don't have tape someplace it shouldn't be, etc...
The only issue I've ever had is some rough handling of my BC during either a tank change, or when I handed up my gear (tank/bc) after a dive. I've seen the DM or captain grab the wing, and when putting the new tank in the "tank slot" the wing has been pinched. My bladder now leaks. I hate to point fingers, but it's hard not to think it could have been prevented...just my $.02
