Leaking from the ambient holes in a piston is almost always due to a leak between the piston shaft and the HP o-ring. While shaft scratches are possible, the more frequent cause is a dried old HP o-ring.
Last possible cause is a leak at the piston head o-ring, but it's is less likely because that's a 135 psi head of pressure, vs 2400 psi at the shaft o- ring.
