This thread reminds me of something I saw when we were at the Sundivers location over Thanksgiving.
While we were having lunch I noticed an older gentleman swimming (breaststroke,even though he had a full set of divegear) back towards the ladder. He looked pretty distressed so I asked a friend of mine who had just got into the water to support him then we both steadied him as he climbed up the ladder.
Turned out he was having a hard time on the dive so his buddies left him to come back in on his own.
He was overweighted (maybe 20 pounds in a shorty) he was scared and he was on his own.

Not sure if it was due to a lack of training/experience or just stupidity but it could easily have had a bad outcome.
