Hey guys and gals,

I am trying to put together a SEABAG form that i can keep in a binder of all the sites that I go to.

I am looking for some help on this, what all I need on there and if there is anything like this that people already use.(no point in reinventing the wheel)

I am also more interested on the first part of the SEABAG for the form, and the second is more of a friendly reminder.

S- Site assessment
E- Emergency plan
A - activity
B- Buoyancy
A - Air
G - Gear and Go

That's pretty much all a general SEABAG is.
The only thing that could be consistent is Emergency Action Plan if all sites are in the same local area."
Everything else will be site dependent. The BAG is essential just do a buddy check and grab your gear before you walk across the parking lot or to the boat. No explanation needed other than that. "Check your BAG and I'll meet you at the beach."

Monterey for instance your EAP should have contact numbers for CHOMP (Community Hospital of Monterey Peninsula), Pacific Grove Hyperbaric, & DAN Emergency Hotline. Then explanation that 911 is priority, then CHOMP, then DAN. PGH essentially isn't staffed until called by hospital; last time I was living there at least.

If you just want bullet points to wing it for a briefing, I wouldn't suggest that. You'll forget to cover a lot. If you're looking to memorize a script for your frequented sites, a dedicated SEABAG for that site will help.
But basic bullet points for my templates were:

Site Assessment:
- Facilities (bathroom, showers, tables)
- Conditions
- Hazards
- Sea Life
- Entry/Exits (primary & alternates)

Emergency:
-----

Activity:
- dependent
