One way to help prevent burst disc corrosion it to fill the hole in the disc retainer with silicone grease or melted wax. It will keep water away from the disc but won't affect operation.
 
Another possibility I have seen is if the burst disc was serviced but replaced with a disc of a lower rating. This could allow for failure in the thin margin which another poster mentioned. I had this happen to one of my twin set cylinders when my trimix instructor overfilled my LP cylinders. I got a new tech instructor.

Burst disc failure is rare, even in cylinders which are subjected to constant cycling in a resort environment. In 4.5 years of working in such environment I saw only the above mentioned failure of a burst disc. No others. I did see 3 or 4 extruded neck o-rings and a few instances of cylinders leaking out the valve assembly.

Sounds like it is time for a bubble test to see where this cylinder was leaking.
