Please suggest a list of simple safety equipment for every diver to carry on every open water dive.
The list should be prominently displayed.
Lost at sea happens far too often.

Rule 1 for open water-PLB. disclaimer, I am probably paranoid because I'm a cave diver, but I was also on a ship where we rescued 2 fisherman in a glorified canoe that had been lost at sea for 3 weeks which doesn't help. Nautilus, Garmin, whatever, but carry one that can actively ping any boat within a reasonable radius. I will not get into anything salty without one.

After that the pretty generic list
Snorkel-I use a rollup in my pocket. Personal comfort at the surface
big f*ckoff SMB, 2m minimum with Solas tape. Color probably doesn't matter much but the solas tape does, doubles as extra inflation.
Shears and/or z-knife for monofilament
 
A Delayed Surface Marker Buoy, DSBM, is probably a good investment. There are other and more affordable signalling devices listed in the previous posts.

Make sure that the DSMB is tall enough (like 7 feet) to be visible from beyond the waves and make sure that it has high visibility SOLAS reflex tape, and that it is closed and can be filled from your BCD inflator on the surface.

For night diving you will need a light.

I can't stand semiclosed dsmb's, do not recommend. Sometimes on the really big ones you don't have much of a choice, but if you can get a closed one, get it.

Here's what I carry on all open-water dives:
1 meter DSMB with over-pressure relief valve on a spool with 100 feet of line
3 meter SMB with SOLAS tape
EMT shears
Knife
Signal mirror
Whistle
C-A-T tourniquet
Sea marker dye
Air horn on my inflator

If I'm in a remote area with strong currents, then I also carry a GPS emergency locator beacon, registered with NOAA.

I'm considering a small square of a space blanket for sun protection while on the surface.

My gear choices are based on my experience in the Navy with searching for people in open water. The signal mirror and sea marker dye are particularly good for making sure that aircraft can find you in open ocean.
