I can't comment on ice diving protocol. I can tell you that scrubber duration should not exceed 3hr on a dolphin and I would like to use less time in extreme cold and work.
I also noticed the use of a 42% mix with a 60% flow orifice which is not condoned, especially in a training environment. That's about all I have to add except sorry for your loss.
