Well, yes, I agree with you Robert... ;)

I was being a bit tongue-in-cheek, since this particular line seems to pop up in almost every fatality or incident report.

Gotta love the "official report" on Wes: "yup, we have no idea".

Complete silence from buddies.

No information on gear.

But we should always just wait for all the official facts to come out, because every time a diver dies, a CSI like William Petersen produces a detailed report on the exact chain reaction of events that led to the death, just like on TV...

With respect, does it matter? You have come out here, which I commend you for, but the question becomes can you live with knowing what you know, not report it because something may be used against you and someone gets hurt or worse at the hands of this instructor?
 
yes, he took a drysuit specialty but used the instructor drysuit ( his size was no longer in stock). Strangely the "instructor" asked him not to touch the valves because he had a tough time "adjusting the valves to his preference"!
The valves function was explained in theory only.
The instructor is only a OWSI for 2 months, and he bought the drysuit as a graduation present for himself. He has very limited drysuit experience himself, and didn't allow my buddy to adjust the valves.

If he is teaching a dry suit specialty without much dry suit experience, then this, too, is a violation.
 
You are right, i have to report these cases to PADI.

I came to the conclussion that i am doing nobody a favor by with holding these facts.

You are required to report these incidents, Jim. Better late than never. If it ever becomes an issue, you can state your reasons for not reporting it earlier, which seem to be that the instructor asked you not to and you felt awkward or intimidated by it; the situations resolved themselves; the multitude of incidents clarified what you need to do as your duty to keep future students and divers safe; or whatever you feel your reasons were for not reporting it right away. If this instructor becomes paired with a less able DM in the future, these types of incidents could very easily become accidents. Why wait?

Not reporting Instructors or dangerous incidents to respective training agencies is part of the complacency regarding the training of divers. Anyone who knows of dangerous practices or incidents that have happened can place a Quality Assurance Report with the respective agency and help ensure that all training produces competent, proficient divers. This will go a long way in avoiding incidents and accidents.
 
Anyone notice that William Petersen looks like Gus Grissom.
 
...Since most accidents have few facts known and any lessons that may be learned come from speculation on why the accident happened from the facts known when the poster gives his thoughts. Some SB members are quite clinical and helpfull, others not so much, but this is a internet fourum not a police report...
Well stated. Speculation basically makes the event somewhat hypothetical, which will still produce much discussion. The discussion itself shows what can or may happen and that itself becomes a learning process.
