I have used Prescription Dive Masks our of California and have had good luck with them. Their website is www.prescriptiondivemasks.com
There are of course plenty of other places to find r/x for masks, but based on experience these people seem to know their stuff.

I have a prescription dive mask from Seavision, they did a nice job and it works great. I don't use it that often as usually wear my contacts.
Both Pearce & I use Seavision bifocal perscription masks. They work great!
