No medical clearance

i personally cannot remember ever being asked for a medical clearance unless i was training. i am not sure of your age but perhaps some shops require a clearance petter for divers over a certain age??
 
On the WRSTC form I have a one minor “yes” answer for RX eyedrops (remember that any prescription med other than birth control or anti-malarial is a “yes”).
Man this ain't flying a commercial airliner with FAA with a microscope up your ass. No way would I be answering yes every time I took a prescription.
 
It would be an uncommon requirement for a dive shop to ask for a signed medical clearance so you could go on a dive trip. Asking you to sign a waiver, maybe; but askin for a full medical clearance, unlikely.
Why are you asking?
Beaches in Turks makes you get an onsite doctor's checkup before you dive unless you check no on every box on their form or have a doctor at home sign the form. You basically lose a full day or more if diving if you have any Rx.

That is exactly why many of us carry a signed form with us, from our annual physical.
Others simply lie on the form and check no boxes.
And some spend the day getting the signature.
 
If there is any concern, I'd get my Doctor's approval letter and get it over with. I don't answer their medical questionnaire since I consider it an invasion of privacy.

I’ve been asked to provide a WRSTC form / medical clearance on an Egyptian liveaboard charter, of all the places, and once on a trip organised by a particularly cautious UK dive shop.

Otherwise never asked outside of a course, same with cards. Just book yourself on a boat, go diving. So the answer is: use a different dive shop or get really good at writing “no”?

Agreed in spirit -- but you're also betting that some lawyer from DAN's underwriter won't be standing there saying "...and this clear evidence that Wheeler925 knowingly lied shows us that they have a disregard for safety that probably led to the accident..."
