Most US dive shops don't require a formal medical clearance document from a physician to participate in regular boat charters or group dives. Some of them require divers to fill out a medical form themselves before participating in certain activities (especially certification courses), and if you mark "yes" for a history of certain medical conditions then they might require a medical clearance.
It would be an uncommon requirement for a dive shop to ask for a signed medical clearance so you could go on a dive trip. Asking you to sign a waiver, maybe; but askin for a full meeical clearance, unlikely.
Not exactly, the places I dive ask for the clearance when ever I took a course and I expected that. BUT, I have been to places, Mexico for 1, that asked for a medical clearance and I am wondering what happen if you dont have one.. I always have one with me, I cant imagine that people on holidays, who just decide to try it have forms with them. what happens? they just get turned away?Your profile says you have been a diver for 6-10 years and are a PADI Master Diver, so it would seem from your question that the dive operators you have dived with have always asked you to bring a medical clearance form, and now you are asking what would happen if you did not bring one. Is that a correct understanding of your question .

Why are you asking?
Try a different dive operator next time you are in Mexico? I suspect the ones that ask to see a completed medical form are the minority. The majority include questions on their liability waiver about your health or fitness to one extent or another, but they don't ask for a completed medical form. You can answer the questions on the liability waiver as you deem best.I have been asked, in Mexico.
