I will be traveling to mexico next tuesday to get certified OW on a PADI course. I know that I will have to fill out a questionare regarding my health. I did have a mild heart attack a year ago.
I just completed my stress tests and a conversation with my DR I asked him if I could dive. He OK'd me. Yea.....
I think I will need a signed form from my DR stating this right?
I need a copy before I go down there. Please.
If anyone has a copy could you PM me and I will send you my fax # or better yet if you have a link to one.

The medical release form with a section for your doctor is there. Just be sure to fill out as much of it as you can before you give it to your doctor, and allow time for his office to complete it and return it to you. (It wouldn't be wise to expect him to drop everything and fill it out while you stand at the reception window.) The forms are in Adobe .pdf format. If you don't have an Adobe reader, there's a link to the Adobe site on the PADI page. (The reader is free.)
