I presume I likely have an inherent increased risk of getting bent in that same spot due to previous damage or my general physiology there? Does that sound likely/possible? Is that the way it works?

Is there anything that can be done am I just going to have to be more conservative?

Is bone necrosis a real risk or is that only really relevant for sat/commercial divers?

before i started doing very long, slow ascent during all my dives, i always
got pain on my right elbow and up to my shoulder. same place, all the time.

i wrote it off to hauling stuff around during dive day.

when i started implementing the long, slow ascents i do now, the pain went
away.

based on this purely anecdotal evidence, it would seem that "getting bent" on
a particular portion of your body may make sense, perhaps an area already
compromised by some physical condition?

you may want to try to extend your stop below where the trouble starts.
if you start getting the pain, go back down and try again. if it's anything
like mine, you can get rid of it if you only take long enough... of course...
you'll have to figure out how this affects your deco profile, etc. etc.
(a problem i didn't have).

Unfortunately, bone necrosis is a real possibility, particularly for divers
who do longer/deeper dives:

The incidence increases with the depth of dives, their duration and the number of exposures. Amateur sport scuba divers were considered to be at low risk because their dives were usually short and shallow, but as they go deeper for longer and use gas mixtures containing helium, it is probable that more cases will come to light.
