Not much "regular" surfing in the islands. The wind surfing is top notch on the north side of Bonaire.

No hiking the jungle in Bonaire - but there is the park on the west end that provides the back to nature opportunity.

Depends on which island (and who you ask).
If you stay at many of the places on Bonaire http://www.buddydive.com/rates.html
the rental is included with your room. Other places are not. I used Buddy's as an example because I knew the web site (having stayed there several times).
Other islands such as Roatan most people would recommend hiring a driver due to the local laws rather than renting a vehicle.
You may want to look at Saba for hiking and the rain forest experience. Fly into Sint Maarten and hop over.
I would still recommend Bonaire ( and definitely rent a car there)!

I think we are leaning towards Grand Cayman and Barbados. Barbados is supposed to have surfing in Bathsheba and pretty good diving, and Grand Cayman I've always heard has great diving. Any suggestions or reasons we should go to different islands for our honeymoon?

Well I am part Hawaiian and my fiancé doesnt want to go there if we can go somewhere else since we are pretty much guaranteed to go there later in our lives. I will have to look at dominica, I assume great diving there?

Dominica is a rain forest type island that has descent diving but does not get alot of press.
I have not been there, so I'm not in the best position to respond.
